Voluntary Surrender of a License

Licensed individuals and firms can surrender an entire license, or specific lines of authority using the Voluntary Surrender form. You must indicate if you are surrendering the entire license or only specific lines of authority. The surrender form must be signed in ink or with a digital signature (Adobe, Windows, DocuSign, etc.).

Some specialty license types, such as Third-Party Administrators and Service Contract Providers, have additional requirements when surrendering a license. Please visit the corresponding webpage for your license type or contact our Specialty Licensing department at (775) 687-0700, option 2 to confirm what is required to surrender your license.

Letter of Clearance

Only use this form if you have been instructed to do so! If you hold a Nevada resident license and are now applying for a resident license in a new home state and are required to provide a Letter of Clearance, complete and return the Request for Letter of Clearance  form to Nevada.Licensing@doi.nv.gov. A Letter of Clearance will inactivate ALL active licenses in Nevada.
