To change the name of a service contract provider, complete the Name Change form and email it to ServiceContracts@doi.nv.gov.
If the company is adding a dba to their name, a copy of the approval of the dba filing at the county level for each county in which the applicant will do business is required and should be attached to the application form. Information regarding filing for a dba is available at www.nvsos.gov.
Name Restriction
The name of the provider may not contain words “insurance,” “casualty,” “surety,” “mutual” or any other word or term that implies that the provider is engaged in the business of insurance or is a surety company; however, the name may include the word “guaranty.” Likewise, a provider may not have a name that is deceptively similar to the name or description of an insurer or surety company or the name of another provider.
How to Update the Owners or Controlling Persons of a Service Contract Provider
To Update the Owners or Controlling Persons of a service contract provider, send an email to ServiceContracts@doi.nv.gov with the requested changes on the company’s letterhead signed by a controlling person or executive.
How to Update the Address of a Service Contract Provider
Service contract providers may update their own Business Location Address and Mailing Address in Sircon. However, the Complaints Address and Secondary Complaints Address must be updated by DOI Staff by sending an email to ServiceContracts@doi.nv.gov with the requested changes on the company’s letterhead signed by a controlling person or executive.
